Skilled Rehabilitation Services

Monticello Nursing & Rehabilitation is best known for our Rehab to Home Program, which is a short-term skilled rehabilitation program where more than 83% of our residents return home!

During your short-term rehabilitation stay you will have access to the best physical, occupational, and speech therapists. You will also be provided a furnished, private suite that includes a private bath, an electric bed, a spacious sitting area where you can enjoy your 42-inch flat-screen T.V. with cable, wireless internet, and a cozy dining area where you can receive private dining service from our professional staff.

Regular care conferences are held with the resident and their family to evaluate rehabilitation progress and keep you abreast of resources available for your transition home once therapies are completed.

You can rest assured knowing you will be able to feel at home while you concentrate on getting stronger.

Outpatient Therapies

Whether getting your strength back following an illness, recovering from surgery, or recuperating from an injury, our outpatient services can provide just what is needed to get you back to enjoying life. We work with all age groups so if you are prescribed therapy and want to stay close to home, ask your physician about completing outpatient therapy at Monticello Nursing and Rehabilitation Center.
